Registered Nurse (RN) – Chronic Disease Management (Permanent, Full-Time)
The RN Chronic Disease Management Nurse reports to the clinical manager and works collaboratively with Edmonton O-day’min PCN family physicians and other interdisciplinary team members to implement primary care network initiatives for patients. This position is based out of the Misericordia Family Medicine Clinic.
RESPONSIBILITIES/DUTIES:
The RN Chronic Disease Management Nurse provides optimal primary care for patients with chronic disease or complex care needs including:
- Supports for diabetic patients, including insulin starts and teaching.
- Asthma, hypertension, hyperlipidemia and COPD support teaching.
- Influenza immunization and other patient injections.
- Assisting family physicians with examinations, treatments, special tests and routine procedures (e.g., assist with routine pap tests) and other duties as required.
- Liaising and care coordination with other PCN team members (e.g., mental health coordinators, anticoagulation management nurses, mental health coordinators, psychologists).
QUALIFICATIONS:
Minimum of 5 years of nursing experience, within a complex primary care or community setting. Please note that all candidates for the above position must have:
- Graduated from an approved Registered Nurse program, accredited through CARNA.
- Home care or case management experience preferred.
- Certified Diabetic Educator preferred.
- Current, active registration with the CARNA.
- Current CPR (Heart and Stroke Foundation BLS Level C- Health Care Provider).
- Demonstrated knowledge of computer software such as electronic medical records, Microsoft Office (including PowerPoint).
- Excellent interpersonal and team skills.
- Clear communication, critical thinking and organizational skills.
